Meetings of the Family Partnership Care Management Coalition are held for the purpose of planning for long-term care redesign in 12 counties. Members discuss design options and methods for delivering the best care for people who will become consumer members.

Consumers and other stakeholders are welcome to attend the full Steering Committee meeting as observers. The meeting agenda will also include 15 minutes for comments by interested parties. Seating will be made available in a section reserved for stakeholders.

Coalition meetings are generally held on the third Wednesday of every month at the Dodge County Office Building at 143 East Center Street, Juneau, Wisconsin. Street parking is recommended.
