The Family Partnership Care Management Coalition (FPCMC) was organized in mid-2005 to begin planning for long-term care redesign. Our coalition grew out of the Family Partnership Initiative, with 15 counties and Lutheran Social Services joining forces to provide managed care and community services for youth at risk of institutionalization.
In planning for long-term care reform, we’re building on the experience of existing, local county long-term care staff and programs, in coordination with community nonprofit partners who have proven experience in high quality, integrated managed care.

There are 12 counties in the FPCMC: Columbia, Dodge, Green Lake, Jefferson, Marquette, Ozaukee, Sauk, Sheboygan, Walworth, Washington, Waukesha and Waushara [FPCMC map].
Community Partners in FPCMC
The nonprofit community-based agencies are Care Wisconsin (CW), Community Care (CC) and Lutheran Social Services of Wisconsin and Upper Michigan.
Two of the agencies – CW and CC – are care management organizations, with long histories of service to the elderly and people with disabilities. Founded here in Wisconsin, these local agencies have provided the Wisconsin Partnership Program as well as other care management programs. Our goal is to ensure that all eligible consumers in the region have access to high quality services. We want to eliminate waiting lists – now totaling over 2,600 for our 12-county region – and support frail elderly and adults with disabilities in their choice to remain independent at home and in the community.
Our planning efforts to redesign our local long-term care system are rooted in the values of choice, quality, access, cost-efficiency and integration. We’re moving toward a fully-integrated approach to care planning and management, where we coordinate long-term care with primary and acute health care.
