Meetings of the Family Partnership Care Management Coalition are held for the purpose of planning for long-term care redesign in 12 counties. Voting members are representatives of those counties and of private agencies who formed the coalition and received planning money. Members discuss design options and methods for delivering the best care for people who will become consumer members.

The activity of planning is a dynamic process, one that involves discussion and change.

It is recognized that parties who are not voting members are interested in the process. They may not only have questions, but also may offer suggestions to further the planning efforts. Those parties are welcome to attend the full meeting as observers. The meeting agenda will include 15 minutes for comments by interested parties and stakeholders.

Because the number of voting participants and their representatives is large, seating for non-members is limited. Seating will be made available in a section reserved for stakeholders.

If interested parties seek further discussion outside of the coalition meeting, they may speak with the representative from their county of residence or the representative from a private agency to learn the contact information for that entity’s long-term care planning contact person.

Coalition meetings are generally held on the first and third meeting of every month at the Dodge County Office Building at 143 East Center Street, Juneau, Wisconsin. Street parking is recommended.
