Family Care is a public program that provides care management for the long-term care needs of eligible individuals. Family Care focuses on the long-term care services and some health services that are part of the benefit package offered by this program. Family Care Partnership is another public program available under the Family Care umbrella. The eligibility requirements for both programs are the same. Partnership covers all long-term care services in the basic Family Care benefit package. In addition, Partnership covers all Medicaid primary and acute medical services as well as all Medicare services, including Medicare prescription drug coverage, for eligible individuals.
Over time, eligible individuals in most FPCMC counties will have access to both programs. Initially, the Family Care program will be offered in all FPCMC counties on a rolling schedule, and the Partnership program will be offered in certain counties to start.
