The Wisconsin Department of Health and Family Services has announced the results from a Request for Proposal (RFP) to solicit entities interested in beginning the process of certification to provide health and long-term care services in 27 counties.  The three community organizations in the Family Partnership Care Management Coalition – Care Wisconsin, Community Care and Community Living Alliance – all responded to the RFP.  Please link below to the announcement from the Office of Family Care Expansion.
